By providing the contact information of the children that did not pass our vision screening at your location, we are able to follow up with the families and make sure that the child gets any needed care or treatment for their vision.
We have developed a simple four-step process where you can submit this data securely using our Excel template. Please see the instructions below:

Enter the referred children’s information in the spreadsheet columns where noted. Please provide as much information in the spreadsheet as possible. There is a tab called “Instructions” that provides further context.

Save the spreadsheet again, to your computer, with the recently entered data included.
