Eye Health Education and Systems of Care for Young Persons with Diabetes in North Carolina
Thank you for taking part in this survey. Prevent Blindness is the nation’s leading eye health and safety organization, dedicated to preventing blindness and preserving sight through education, advocacy, community and patient services, and research. This survey is part of a project focused on improving eye health for children and young people with diabetes. We want to better understand the protocols and systems clinics and community health centers use to manage diabetes in pediatric and young adult patients, particularly as it relates to eye health. Your insights are incredibly valuable and will help us identify best practices and areas where additional support may be needed. A summary report of findings will be shared with participants after the project concludes.
